Aggi Kozlowska Enterprises Inc. (“Aggi Kozlowska Enterprises”) respects and is committed to protecting your privacy. This “Privacy Policy” governs your access to and use of the websites www.aggikozlowska.com (and/or any sub-domains of www. aggikozlowska.com or www.agatahealth.com) including any content and services offered on or through the websites and any social media accounts operated by Aggi Kozlowska Enterprises (the “Websites”), and summarizes what personal information we collect, and how we use and disclose this information. This Privacy Policy applies to both visitors to the Websites (“Non-Registered Users”) and to those that register with the Websites (“Registered Users”). Age Restrictions

The Websites are intended for adults over the age of 16. Only individuals over the age of 16 may provide information to our Websites. Aggi Kozlowska Enterprises will not knowingly collect personally identifiable information from individuals under the age of 16. If you are under the age of 16, please do not provide any Personal Information or Personal Health Information to us. Aggi Kozlowska Enterprises may, at her sole discretion, require users under the age of 16 to obtain consent of a parent or legal guardian to view certain content, and certain content may be restricted to users over 16 years of age. Please let us know if you have reason to believe that an individual under the age of 16 has provided Personal Information or Personal Health Information to us and we will take steps to remove that information from our database.

Personal Information

Personal information is any information that identifies an individual.

Personal Health Information 

Personal Health Information is defined by the Personal Health Information Protection Act, (2004, S.O. 2004, c. 3, Sched. A) (“PHIPA”) as any means identifying information about an individual in oral or recorded form, if the information,

(a) relates to the physical or mental health of the individual, including information that consists of the health history of the individual’s family,
(b) relates to the providing of health care to the individual, including the identification of a person as a provider of health care to the individual,
(c) is a plan of service within the meaning of the Home Care and Community Services Act, 1994 for the individual,
(d) relates to payments or eligibility for health care, or eligibility for coverage for health care, in respect of the individual,
(e) relates to the donation by the individual of any body part or bodily substance of the individual or is derived from the testing or examination of any such body part or bodily substance,
(f) is the individual’s health number, or
(g) identifies an individual’s substitute decision-maker.
Aggi Kozlowska Enterprises collects, uses and discloses Personal Health Information in accordance with PHIPA. 

What Personal Information and Personal Health Information We Collect

Aggi Kozlowska Enterprises collects Personal Information and Personal Health Information provided orally, through email, fax, in person, or through any other means of communication to Aggi Kozlowska Enterprises. For example, we may collect Personal Information and Personal Health Information when you: • Register as a client or as a user for the Websites (your address, phone number, email address, financial information, social media accounts etc.); • Filling out forms as a client, including, but not limited to, information about a client’s health history, family health history etc.; • Participating in any surveys; • Enable location services on your computer or electronic device; • Contact us through email, telephone, mail or other correspondence; • Ordering information when purchasing products and/or services to fulfil your order; • Register for a service we provide; • Register to receive our newsletter; • Search or request information on the Websites. The Personal Information we may collect will depend on the service you use and may include your full name; metadata, contact information, such as your address, telephone number, or email address, and any other Personal Information and Personal Health Information that you choose to submit to us. We also automatically collect personal information about you indirectly about how you access and use the Websites and information about the device you use to access the Websites. If you chose not to provide Personal Information or Personal Health Information, we may not be able to provide the services to you or respond to any other requests. 

Billing Information

Aggi Kozlowska Enterprises may also collect personal information when you make purchases online, in addition to the Personal Information and Personal Health Information noted above. The information you provide to make purchases (for example, credit card number, billing address, shipping address etc.) will be shared with payment processors, financial gateways and your credit card company to authorize credit card payments. This information may also be shared with necessary third parties to carry out the transaction.

How Aggi Kozlowska Enterprises Collects Information through Technological Means

How Aggi Kozlowska Enterprises Uses and Discloses Personal Information

The Personal Information we collect may be used by Aggi Kozlowska Enterprises for the purposes for which it was collected, as provided in this Privacy Policy, or for other purposes that are disclosed to you and to which you consent.   • Analytics and search engine providers (e.g. Google Analytics) that assist us in the improvement and optimization of our Websites by collecting data and information. If you opt out of the search engine provider’s analytics collection, you may not be able to use the full functionality of the Websites; • Invoices for goods and/or services and payment processors for the purpose of fulfilling relevant payment transactions;

  • Third parties in connection with a business transaction. Personal information may be disclosed to third parties in connection with a transaction, such as a merger, sale of assets or shares, reorganisation, financing, change of control or acquisition of all or a portion of our business. Personal Information we collect is maintained with the source of the information on the Cloud. We retain Personal Information that we collect only as long as necessary for the purposes for which it was collected or to meet legal requirements. We destroy Personal Information when it is no longer needed.

Storage and Transfer of Your Personal Information

The information you provide to us is stored on secure servers in a controlled environment with limited access. Any payment transactions will be encrypted in transit using SSL technology. If you have a password which enables you to access certain parts of our Platform, you are responsible for keeping this password confidential and you should not share your password with anyone. 

While no transmission of information via the internet is completely secure, we take reasonable measures to protect your personal information. Once we have received your information, we will use strict procedures and security features to try to prevent unauthorized access. We cannot guarantee the security of your personal information transmitted to our Platform and any transmission is at your own risk. Aggi Kozlowska Enterprises may feature public forums, such as message boards, chat rooms, webinars etc. (“Public Forums”) where you may voluntarily share information. Information that is shared through the Public Forms is NOT protected by this Privacy Policy, including any Personal Information or Personal Health Information. Information provided through the Public Forums is public information and may be viewed, used, modified and/or disclosed for any purpose any by anyone, including the Public.
